CPHealthy and the St. Lawrence Health Initiative Present…..

Are you at Risk for Diabetes?

Take Charge of Your Health!

You can prevent type 2 diabetes with this free year-long program!

4 class options:

  • Every Wednesday starting March 1 from 1-2pm or 5-6pm

at the EJ Noble Building, Betty Evans Room, in Canton

  • Every Thursday starting March 2 from 3:30-4:30pm or 4:45-5:45pm at the 20 Cottage Street Conference Room, in Potsdam

You can prevent type 2 diabetes:26 weekly sessions give you the tools you need to eat better and be more active. Supported by the other participants, and coached by a certified diabetes coach, you will develop lifelong skills for healthy living, by focusing on step-by-step change. You will work towards a goal of losing 5-7% of your starting body weight and reaching and maintaining 150 minutes of moderate physical activity each week. There will be bi-monthly check-ins to help you keep on track and maintain your progress after the weekly program ends.

You may be at risk if you:

  • Had a baby weighing more than 9 pounds at birth OR had gestational diabetes during a past pregnancy
  • Have a sister, brother or parent with diabetes
  • Are overweight
  • Get little to no exercise in a typical day
  • Are 45 years of age or older
  • If you’re not sure if you’re at risk or if you have pre-diabetes, you can take a quick self-assessment at www.gethealthyslc.org. Just scroll down to the very bottom of the website’s home page for the quiz.
