The Lifestyle Program Training and Technical Assistance Center of the North Country provides training and technical assistance for delivery organizations located in Northern New York. The Center employs Master Trainers for the Chronic Disease Self Management Education and the National Diabetes Prevention Program. The Chronic Disease Self-Management Education (CDSME) programs are a six week interactive peer-support program designed by Stanford University, and currently licensed by the Self Management Resource Center. The workshops focus on issues common to people with a variety of health conditions; participants develop skills through Action Planning and Feedback, Behavior Modeling, Problem-Solving Techniques, and Decision Making.

Participants will receive the Living a Healthy Life with Chronic Conditions published by the developers of the Self-Management Programs. This book is a vital resource full of practical tips, suggestions, and strategies to build confidence in managing chronic illness and symptoms. Also offered is the “Relaxation for Mind and Body”, relaxation CD, designed to accompany the resource book.

The National Diabetes Prevention Program (NDPP) is a year-long program designed to assist participants in a lifestyle change for the purpose of preventing diabetes. 
Centers for Disease Control (CDC) recognizes local delivery partners.
If you have prediabetes, getting type 2 diabetes doesn’t have to be a sure thing. In fact, prediabetes can often be reversed. You’ve decided to make some healthy changes. Prevent T2 is a CDC recognized diabetes prevention program developed specifically to prevent type 2 diabetes. The program can also lower your risk of having a heart attack or stroke, improve your health overall, and help you feel more energetic. It’s designed for people who have prediabetes or are at risk for type 2 diabetes, but who do not already have diabetes. A trained lifestyle coach leads the program to help you change certain aspects of your lifestyle, like eating healthier, reducing stress, and getting more physical activity. The program also includes group support from others who share your goals and struggles. This lifestyle change program is not a fad diet or an exercise class. And it’s not a quick fix. It’s a year-long program focused on long-term changes and lasting results. A year might sound like a long commitment, but learning new habits, gaining new skills, and building confidence takes time. As you begin to eat better and become more active, you’ll notice changes in how you feel, and maybe even in how you look. You may be at risk if you: • Are 45 years of age or older • Are overweight • Have a family history of type 2 diabetes • Are physically active fewer than 3 times per week • Ever had diabetes while pregnant OR had a baby that weighed more than 9 pounds. What you can expect: • The skills you need to lose weight, be more physically active, and manage stress • A trained lifestyle coach to guide and encourage you • Support from other participants who share your goals – and expect to have fun!!! • Weekly meetings for the first 6 months, twice monthly for the second 6 months
