The Chronic Disease Self-Management Education (CDSME) programs are a six week interactive peer-support program designed by Stanford University, and currently licensed by the Self Management Resource Center. The workshops focus on issues common to people with a variety of health conditions; participants develop skills through Action Planning and Feedback, Behavior Modeling, Problem-Solving Techniques, and Decision Making.
The National Diabetes Prevention Program (NDPP) is a year-long program designed to assist participants in a lifestyle change for the purpose of preventing diabetes.

Participants who participate in the Chronic Disease Self-Management Education will receive the “Living a Healthy Life with Chronic Conditions” published by the developer of the Self-Management Programs.  This book is a vital resource full of practical tips, suggestions, and strategies to build confidence in managing chronic illness and symptoms.  Also offered is the “Relaxation for Mind and Body”, relaxation CD, designed to accompany the resource book.

 If you have a patient with prediabetes, getting type 2 diabetes doesn’t have to be a sure thing. In fact, prediabetes can often be reversed. A CDC recognized diabetes prevention program – Prevent T2 is developed specifically to prevent type 2 diabetes. 

The program can also lower a patients risk of having a heart attack or stroke, improve their health overall, and help them feel more energetic. It’s designed for people who have prediabetes or are at risk for type 2 diabetes, but who do not already have diabetes. 

A trained lifestyle coach leads the program to help patients change certain aspects of their lifestyle, like eating healthier, reducing stress, and getting more physical activity. The program also includes group support from others who share similar goals and struggles. This lifestyle change program is not a fad diet or an exercise class, and it’s not a quick fix, it is a year-long program focused on long-term changes and lasting results. 

Patients may be at risk for diabetes if they:

  • Are 45 years of age or older
  • Are overweight
  • Have a family history of type 2 diabetes 
  • Are physically active fewer than 3 times per week 
  • Ever had diabetes while pregnant OR had a baby that weighed more than 9 pounds 


Patients who enroll in the program will learn the skills they need to lose weight, be more physically active, and manage stress.

A trained lifestyle coach will guide and encourage all participants. 

Patients receive support from other participants who share their goals. 

The program is designed with weekly meetings for the first 6 months, then twice monthly for the second 6 months.
